Jessica, Jeanette, and Alexia are absolutely fantastic to work with! I’ve had the pleasure of working closely with them for the last 6 years and they are all top notch. They are responsive, person centered, and have a heart for service. I can’t recommend them enough! Wonderful group of people.
There are some of the most caring staff out there who love what they do. Management and floor staff make sure that your stay is a good experience. Caring, compassion and comfort are always #1.
They seem pretty attentive to the residents there and they are taking good care of my uncle, but there are a few of them that are slow with the administering of meds, because my uncle had said that he had to ask several times for his meds before they would give it to him.
The therapy team here is amazing. Working with Jenny was great I’m only a week and a half post knee replacement surgery and they’re helping me so much and they’re so positive and so patient. If you need therapy, I highly recommend you go see these guys. I really can’t believe how far I’ve come already and I know it’s because of their diligence and making sure I’m doing what I’m supposed to do and doing it right.
I’m a patient here being discharged today at cascade terrace and I was impressed with the therapy department feeling like I actually did something with my body and feel so much stronger. The team here was amazing to work with. Loved working with Christian, Jenny, Elynn helping me break my fears of going up and down curbs and I feel like I can do that better. The kitchen had a bomb chef salad too.
Everyone was excellent, kind, caring & courteous! The therapy team is Justin, Kendra, Kenny, Christian, Christina, Jenny & Elynn are awesome!
I was admitted to CASCADE TERRACE POST ACUTE 10/14/25 to 11/03/25, for rehabilitation after I broke my femer. The staff were nice and attentive. I had some issues with a resident hounding me for cigarettes, and calling me names when I told him no numerous times, so I told the staff. Otherwise my stay was mostly pleasant.
Employees here are amazing, patients and families are loved and well cared for. You couldn’t ask for a more dedicated group of professionals. Highly recommend!
This place is very gross and please anyone that disputes this just ask to take a tour by contacting the admissions coordinator. I suggest just dropping in and ask for Blake because he’s acutely aware of the filth, the smell and the danger to patients and employees this unkept building poses, in fact he is always in his office usually with the door shut do to the smell and not wanting to be visited by patients uninvited. When asking for Blake the administrator to take a quick tour please pay attention to the office staffs faces and mannerisms..I’m sure they will try and pawn you off to someone else for a tour or make you schedule a day further out so they can try and cover-up what they can, there’s no covering up the foul 24/7 unhygienic smell. Patients use call buttons that don’t register correctly due to the facilities WiFi not connecting to half the building so patients are not helped and miss needing medications and so on. I have attached a few pics of dangerous uneven floors and broken tiles patients and staff trip on and slip causing harm, but is covered up as something else. I’m sharing pictures of sunken in floors and floors so rotted the are mush under your feet and sharing pics of mold and filth. While visiting this facility take a look up at the ceiling and you can see areas they tried painting over mold stains, you’ll see ceiling tiles missing or crooked due to not fitting properly, you’ll see water stains from leaking ceilings, take a look. At the fire sprinklers and the thick rust that’s incased. I could go on.. please understand some of my pics cannot be shared with the public but beleive me I’ve shared with the proper authorities. These pictures and others of this facility have been found in FB groups and video on YouTube but its the communities responsibility to still share as much as possible and draw attention to this these folks live in our community and are vulnerable so lets show we care. I was admitted on a Friday and wasn’t seen by PT until the following Monday for a PT evaluation. I was discharged by Emanuel hospital with my limitations and safety needs. I was left over the weekend without a walker. I am a 1 person assist with walker. I was diagnosed in the hospital with functional neurolocal disorder. Nobody assisted me with any care and I was left to do it on my own. I requested to talk to the nurse and let her know I needed a walker and what I needed for assistance by the CNA’s. She told me they couldn’t give me a walker or wheelchair without a PT evaluation. She then proceeded to tell me not to fall and left. It took forever at times to get my meds especially my nausea meds they were as needed, well over 2 Hours including my pain meds that day. That following morning the med aide came saying she had my morning meds and I asked what they were she rudely told me they were my morning meds and finally told me what I was taking.The following night they forced me to leave against my will the night before the nurse came to check my catheter because it wasn’t draining properly he told me it was fine and asked if I pulled on the tube, I told him no and that continued the rest of the night. He told me he would come back to check on me and just went home. That morning the day shift CNA came in and I let her know what was going on with my catheter and she went for help nobody came. I finally called the social worker and she got the DNS and nurse and they figured out my catheter wouldn’t flush and they sent me to the hospital to get help. The hospital found other medical issues going on. I returned to the facility and the night nurse from the night before came in my room saying my medical issue I was sent to hospital wasn’t the problem with my catheter, I reassured him that wasn’t the case. I let him know how he made me feel that my care I got from him wasn’t adequate or fair treatment and he just left me with my bladder retaining urine he dismissed my feelings and concerns and walked out. My husband came to the facility and brought my meds he went to talk to the nurse to let him know the care he provided was poor and unfair. My husband tried to talk to him in private so his coworkers didn’t hear. He said my husband threatened him by his frustration and called the cops. My husband waited for the police. The DNS of the facility told the cops she was discharging me and 86ing me and my husband off the property. The DNS and another nurse came into my room with a box and I asked what was happening she said hunny your leaving. I said why I didn’t do anything and the DNS said your husband isn’t on allowed on the property and started packing my things without my permission. I told her to stop and she instructed the nurse to finish packing my things and put them on the cart. I told her I need to be her to get stronger and she said hunny you don’t want to be here if your husband isn’t allowed and processed to push my things out on the cart. I had no choice but to follow my things outside. I asked her for the administrators phone because I have patient rights and she told me hunny you left against medical advice. I told her I absolutely didn’t do that you forced me out. This DNS told the police officer she was discharging me and trespassing me off the property before she pushed me out of the room. This was the most traumatic experience I have ever had regarding my health and well being. This place isn’t the place for anyone needing proper care and love from their medical providers or their loved ones.

Our father had a near death experience that resulted in him being transferred to Cascade Terrace rehab before going home. At Cascade Terrace He was treated extremely well with the professional care he required and needed with love and kindness. We’re so grateful and grateful for the guidance of the staff at Cascade Terrace. We all came to the conclusion that our father needed to go to assisted living and Cascade Terrace was instrumental with our father regaining his health and mobility. Cascade Terrace is also instrumental in pointing us in the right direction to assisted living that our father needed. I cannot say enough good things about Cascade Terrace in the care our father received!!! Thank so you very much Cascade Terrace A+++
There’s new management as well as implementation of new programs like PT/OT respiratory and speech for the first time in skilled nursing in oregon!

This facility was recently acquired by a new medical group and there was an administrative management change. The new management is awesome and is really pushing to make needed changes
My mom had stroke was sent here from OHSU stuck there for months, mistreated filed complaint thru state many wrong doings were found!! Tried placing my mom in nut ward…Mind you she was perfectly fine before stroke only problem was them keeping her so dumbfounded by many different kinds of downers! My mom complained they were mean to her mistreated her and Monica one of there nurses/Workers hit my mom thru phone at her…Will be taking further steps..Finally got paperwork from all findings and faults found on them…BAD PLACE WOULD NEVER RECOMMEND money was stolen from her box…I was buying her new clothes each week kept finding them to be missing and found dirty molded clothes hid in bottom of her closet got many pictures for proof..
